ENTERTAINMENT AGREEMENT

The party’s agreement is for Music 1 Disc jockey services for the event
described below,
between the undersigned Client and Music 1 Disc Jockey Services.
Music 1 Dj Services agrees to furnish services to the following client and
specifications as listed:

Payment terms: The total fee agreed upon by Client and Music
1 Dj is $___________ including tax.
To confirm this agreement and to hold event date sign and return this copy
along with a deposit of $100.00 dollars
or you may pay in full to the above address,
If you elect to only pay the deposit the balance will be due 30 days prior
to the event.
If you cancel or balance not paid at the elected time your deposit is not
refundable.
There will be a $40.00 fee charge for all returned checks. All checks are
payable to P.D. Brittain.
If you need to make other arrangements on payments call me and we will set
up payments schedule.
Gratuities given to the Disc Jockey at the event are made at the discretion
of the client.
Cancellation: This agreement cannot be cancelled or modified except
in writing by either the client or Music 1 Dj.
Rescheduling for events canceled due to inclement weather shall be accommodated
whenever possible.
Rescheduled events are subject to availability.
If cancellation is initiated by client in a timely manner at least 90 days
prior to event deposit will be refunded,
however if cancelled at short notice the client is responsible for the total
fee.
Overtime Cost: If feasible, We will allow client to extend
the music playing time to accommodate their event,
The cost is a rate of $______per hour and is due on the day of the event,
at this time cash will be the only method accepted.
It is the client responsibility to have permission to extend the music at
the facility they have selected.
Providing Services: The scheduled Dj personalizing the event
will be P.D. Brittain and his assistant Darlene.
In an unavoidable circumstance that Music 1 Dj would not be able to perform,
a substitute of the similar experience would be provided for your event date
and time.
Clients Responsibity: The client shall ensure that the facility
they selected will provide proper area to set up equipment
and proper amps for the said equipment and lighting.
That the facility meets all federal & state requirements and has any
required permits.
The facility is open 2 hours prior to allow us set up time.
Any outdoor events must have proper shelter so that equipment is out of weather
and additional electricity for outdoor use.
If possible have facility to provide us with one table.
Upon signing of this agreement the client accepts full responsibility and
is liable for any damages, injuries,
or delays that occur as a result of failure to comply with these provisions.
In the event that a threaten situation or implied threat should occur
the performance would cease until situation is under control,
any such damage to equipment or recording would be client responsibility.
Music 1 Disc Jockey services reserves the right to deny any guest or client
access to recordings or equipment.
Upon receiving your deposit and signed contract if we have viability for the
said date
a copy of payment along with copy of contract/agreement confirming your date
will be sent to you.
